The aim of this work was to design and development of reference systems intended for operational records information on institutions Chernivtsi region. Work carried out on the request of Postgraduate Education, Chernivtsi Oblast. The subject of the research is to design and practical development of records management information system based on MySQL database server and server-side scripting language PHP. Object is a method of designing and developing information system operational records information on institutions Chernivtsi oblast and software algorithms that were used to develop the information system. In the research work covers theoretical approaches and methods of designing and developing application information and reference systems based on the database server MySQL and server-side scripting language PHP. Based on the terms of reference established logical and physical database model, in which all registered schools area and identification information provided to representatives of these institutions. Developed custom, customer and administrative part of the information system. Created elements of information systems available on the Web-server Institute of Postgraduate Education, Chernivtsi Oblast (http://isuo.ippobuk.cv.ua/). Tools for summarizing data stored in the system, and export files in the format of Microsoft Excel.
