S., Registration Date 27-01-2000 Organization V.P.Komisarenko Institute of Endoсrinology and Metabolism popup.description2 The object of this study were type 1 and 2 diabetic patients with diabetic angioneuropathy, mice with streptozotocin diabetes. The aim of the study was to develop new methods of treatment of diabetes mellitus and its complications. Clinico-biochemical, radioimmunological, instrumental methods of investigation have been used in this work. In searching ways of optimization of treatment of diabetics it has been shown, on the basis of the results of prospective investigations, that in most patients receiving both repeated and traditional insulin therapy without appropriate control, diabetes is in decompensation state. In case of repeated insulin therapy the degree of decompensation is less pronounced than in case of traditional one, what contributes to a somewhat more favourable course of diabetic angiopathy. A new oral hypoglycemic drug - Repaglinid (Novo-Norm) -was found to be more adequate and effective for therapy of type 2 diabetes as compared to existing drugs, as regulator of postprandial hyperglyce mia. Preparations of alpha-lipoic acid (Espa-lipon and Berlithion) were found to be effective for treatment of diabetic polyneuropathy. Use of quanta hemocorrection and microwave resonance therapy yielded positive results in patients with diabetic angioneuropathy (retinopathy). Microwave resonance therapy contributes to a normalization of processes of glycosylation of blood proteins. Experimental grounds have been obtained for clinical use of new drugs which influence the main pathogenetic links of diabetic polyneuropathy: a) blockators of calcium channels in patients with sensor neuropathy in order to normalize the disturbance of intracellular calcium homeostasis, which we have revealed; b) vegetal lectin ConA and WGA which stimulates the synthesis of glycosaminoglycans in tissue cultures by influencing one of the pathogenetic mechanisms of angioneuropathy: disturbed metabolism of intercellular tissue matrix in diabetic patients. The results obtained have both theoretical and practical impact; they provide n ew data on pathogenetic mechanisms of development of diabetic angioneuropathy; on their basis new, more effective methods of treatment of diabetes mellitus and its complications have been proposed.
