To elucidate the incidence and terms of tolerance development. To study some biochemical criteria as well as to elaborate the methods of prevention and the ways of correcting the nitrates tolerance development in antianginal treatment of IHD patients aged over 60.Participants: The elderly patients (age range, 60 - 74 years) with chronic IHD, a stable angina pectoris of II, III, and IV functional classes. Methods: Clinical observation, bicycle ergometry, tetrapolar rheography, 24-hour ECG monitoring, assessment of plasma lipids and indices of free radical oxidation system, determination of the level of the stable metabolite of nitrogen oxide (NO2), temporal analysis of the heart rate variability, measurements of catecholamines excretion in urine, evaluation of adrenaline-induced platelets aggregation. Results: The nitrosorbide (isosorbidedinitrate) tolerance was shown to develop in 15 % of the patients inv estigated in the 5th and the 10th week of continuous treatment with nitrosorbide in a daily dose of 30 mg. It have been found out the factors predisposing to nitrates tolerance development in the patients with IHD. It has been established the interdependence between the development of nitrates tolerance in elderly patients with IHD and activation of free radical oxidation system, the deterioration of cGMP synthesis, SH-groups deficit, and activation of sympathetico-adrenal system. It has been suggested a method of tolerance development prediction. Its essence consists in the concurrent administration of captopril, an ACE-inhibitor, and vitamin E, an antioxidant, on the background of treatment with nitrosorbide. Novelty: It has been established some diagnostic markers of the development of nitrates tolerance and elaborated the methods of preventing the tolerance development in elderly patients with IHD by means of concurrent administration captopril, an ACE-inhibitor, and vitamin E, an antioxidant, on the back ground of treatment with nitrosorbide.
