The purpose of work: increase of efficiency of treatment and rehabilitation, improvement of quality of life of victims with colostomy concerning damages of a thick gut, on the basis of a substantiation, development and introduction of the specialized clinic-rahabilitation program. . Methods of research: the retrospective clinic-statistical analysis of an archival clinical material; perspective studying of parameters of quality of life in dynamics a method of biographical testing; clinic-laboratory; clinic-tool, endoscopy, functional and histologic researches of a condition of a thick gut; parametrical and nonparametric methods of the statistical analysis. Results of treatment of 751 victims with damages of a thick gut are investigated. From them 43 - with gunshot wounds, 325 - wit h prick-cut, 383 - with the closed stupid traumas. The special attention is given preoperative to diagnostics of damages of a thick gut, forecasting of an opportunity of application colostomy and to definition of an optimum place of a conclusion of the future ostomy on a forward belly wall. The applied method of an estimation of quality of life. It scientifically has allowed to prove the reasons of the unsatisfactory direct and remote results of surgical treatment of damages of a thick gut to define(determine) ways of their improvement. The specialized treatment-and-rahabilitation program which provides application of rahabilitation actions from the moment of damage of a thick gut before performance of reconstructive -regenerative operative intervention and returning of the victim to public life is developed. Introduction of actions of the program has allowed to execute preoperative diagnostics of damage of a thick gut in 62,9 % of victims against 31,3 % in control group to predict an opportunity of performan ce colostomy in 86,4 % of patients against 53,3 %, has excluded inadequate formation colostomies, has lowered mortality with 54,2 to 37,1 % and has improved quality of life in 1,4?0,2 time.
