Objective: working out a system for prodicting DM complications in children and adolescents, and improving the methods for theiz prevention. Methods: conventional clinical, clinical genealogic, instrumental, biochemical, immunogenetical and statistical. Theoretical and practical results: clinical features of DM in children and adolescents at different stages of the disease were investigated, activation of lipide peroxidation processes against the background of metabolism decompensattion and ketoacidosis. It was established thad duration of the disease and its unsatisfactory compensation as well as DM manifestation in prepubertal age and existence of certain HLA-haplotypes DR3DR4, DR2DR3 and DR5DR7 were integral risk factors of DM severe course and development of complications in children and adolescents. Specific risk factors were established for certain DC: disbalance betw een activity of lipide peroxidation processes and the system of antioxidant protection functioning - for angiopathy; pubertal period, especially in girls - for nephropathy and decrease in vitamin E level - for sexual development delay. Novelty: certein regularities in the development of DC at different stages of the disease as well as the role of genetic factors in such complications were studied. A system for prognosing severe course of DM and predicting manifestation of DM early complications in children and adolescents were worked out taking into consideration anamnesis, certain findings of lipide peroxidation, activity of antioxidant protection and genetic factors. Stage-by-stage approach to the treatment courses of DC considering the risk factors of their development was proposed. Fieds of use: endocrinology, pediatrics.
