On the basis of complex investigation of rheological properties of the blood and functional state of vessels endothelium in elderly practically healthy people and chronic IHD patients of the same age, the present study seeks to determine the significance of microcirculatory changes in the development of circulatory hypoxia and to elaborate the methods of correcting hypoxic shifts in these categories of population. The study comprised 231 persons aged 68-74 of whom 45 subjects were healthy people and 184 persons were the patients with chronic IHD. Besides 22 practically healthy subjects aged 20-29 served as controls. Thus 253 people were investigated on the whole. Methods of investigation. Clinical examination, laser doppler flowmetry, aggregatometry, viscosimetry, polarography, 24- hour ECG monitoring, 24- hour cardiorhythmography. Results. It has been drawn a theoretical general conclusion on the role of capillary blood circulation deterioration in development of circulatory hypoxia with physiological ageing and in elderly patients with IHD. It has been established a correlation between the state of microcirculation and episodes of myocardial ischaemia, which are not associated with an increase in myocardial oxygen demand, and the role of autonomic dysbalance in development of circulatory hypoxia in elderly patients with IHD has been determined. The differential approach to administration of drugs with a preferable effect on individual elements of microcirculatory bed and haemorheological system has been substantiated and it has been shown the possibility of their combined administration in order to improve the oxygen provision of tissues in elderly patients with IHD. Novelty. It has been determined diagnostic criteria of circulatory hypoxia development associated with detcrioration of blood flow in microvessels in elderly healthy people and IHD patients of the same age. Some new methods of assessing the functional state of endothelium and platelets as well as a method of cerebral blood flow assessment in elderly people are suggested. The possibilities of correcting the oxygen provision of tissues in elderly people by drugs having an effect on microcirculation are determined. Introduction forms. The patents are taken out for methods of determining the functional state of endothelium and platelets. Besides a patent is applied for a method of assessing the state of cerebral blood flow. The information letters concerning the peculiarities of clopidogrel, ticlopidin and enalapril administration in elderly people are published and the possibilities of differential administering the antiaggregants to elderly patients with IHD are recommended.
