"Zemstvo" was the special organizational system of local community self-rule infroduced on rural territories of the former tsarist Russia in the second half of the XIX-th and beginning of the XX-th centure. It was gradually introduced on the Left-river side and South part of the Ukraine in 1860-es and also in 1911 on the Right-river side. The comperative analysis of the main characteristics of "Zemskaya medicine" and its performancy indices in these regions of Ukraine at that time has been made. It includes its medical facilities network, its medical stuffs' composition, effectiveness of facilities functioning comperatively with the preceded period, etc. The advantages of the "Zemskaya medicine" in comparison with the one administratively imposed (s.c. "released from above orders" medicine) have been revealed. The most important features of "Zemskaya medicine" were introduction in its facilities (structure)such principles as overall accessible and free primary medical aid provision, its districts-level's infrastructure organization and prophylactical orientation. These principles have been preserved in the Ukrainian PHS up-to-now. The study has also presented some data which characterized Ukrainian population health status/conditions at that time and its dynamics in the "Zemsky" and alternatively in "non-Zemsky" guberniyas (the former large-scale administrative-territorial division in the former Russian Empire).
