To find out the prognostic value of non-invasive indices characterizing anatomical electrophysiological and functional status of the myocardium for diagnosis of the risk of heart rhytm disorders in the elderly IHD patients. Subject of inquiry. The patients with IHD (stable angina pectoris, functional class I-IV, postinfarction cardiosclerosis, heart failure) Methods. High resolution ECG, surface ECG with assessment of amplitudes and dispersion of intervals and waves 24-hour ECG monitoring, RR intervals, echocardiography). Major results. The data obtained testify to the fact that the frequency of ventricular extrasystoles in elderly IHD patients significantly correlate with extent of QT intervals dispersion, of the QRS wave, time of 24-hour ischaemia, ejection fraction and presence of ventricular late potentials. With the help of a set up equation of step-by-step multiply regression it has been established that the ejection fraction and the extent of QT intervals dispersion are the most significant indices for finding out the risk of ventricular extrasystolic arrythmia development. While assessing the risk of atrial fibrillation in IHD patients aged over 60 a special attention should be paid to the state of autonomic control, the presence of atrial late potentials, morphofunctional state of the left atrium. The left ventricular and left atrial ejection fractions and the level of blood pressure in the pulmonary artery are of the most predictive value. For exploring the risk of frequent atrial extrasystoles it is expedient to study the functional state of left atrium and left ventricle, the presence of atrial late potentials and time of 24-hours myocardial ischaemia. Among the above indices the size and the volume of left atrium and the presence of atrial late potentials are of the most predictive value.
