The purpose of work - to study a role of neurohumoral mechanisms of regulation of vascular tone in development and progression of CHF, to develop the differential approaches to diagnostics, treatment and preventive maintenance of CHF depending on activity of neurohumoral systems of vasoregulation. The research methods included definition of the the vasoactive mediators levels by radioimmunoassay/immune-enzyme assays, echocardiography, methods of variational statistics. As a result of research it was marked, that the high levels of adrenalin, noradrenalin, angiotensin II, aldosteron, ANP and endothelin-1 concern to "malignant" criteria of CHF course. The high clinico-hemodynamic efficiency of enalapri , metoprolol and losartan in the patients with CHF was determined. It was shown, that metoprolol had the most pronounced effects on sympato-adrenal system, losartan - on rennin-angiotensin-aldosteron and antidiuretic systems, whereas enalapril appeared to most effective in regards to the levels of ANP, insulinresistance and endothelial function. According to features of neurohumoral mechanisms of vasoregulation the differential schemes of enalapril, metoprolol and losartan administration were developed.
