Gorky popup.description2 Goal: To study possibility of Fetal Pulsoximetry use, peculiarities of neonates status and possibility of neonatal asphyxia prognosis with Fetal Pulsoximetry, to work out Fetal Pulsoximetry Protocol and to estimate its effectiveness in the Pilot sites. Methods: clinical, physiological (US, Fetal Pulsoximetry OBScientific), biochemical (Gas analyzer IRMA 2000, BG cartridges), qualitative research (interview), statistical (non-parametric). Materials: 24 pregnant women during delivery and their neonates at the first phase of Project, 554 case records in the Pilot sites at the second phase. Field of use: medical facilities of the III-IV level of accreditation. Results: Decline of fetal saturation lower that 30% correlates with neonatal asphyxia and consequent hypoxic-ishemic disturbance of CNS. More frequent use of Fetal Pulsoximetry for fetal monitoring (36% versus 13%) in the framework of proposed Protocol has led to significant increase in neonate Apgar score at the first and fifths minute after delivery. Use of proposed Fetal Pulsoximetry Protocol and consequent actions regarding data obtained has decreased Cesarean Section rate significantly (p<0.05). Fetal surveillance becomes objective and groundless Cesarean Sections declines. Implementation of proposed Fetal Pulsoximetry Protocol for fetal monitoring can improve quality of care provided for mothers and neonates.
