Based on the analysis of long-term results, indications and course as well as tactics of surgical management and the surrey of posoperative period are developed. Indications for splenectomy in patients with chronic lymphoproliferative disoders include splenic enlargement with compressive syndrome, splenomegaly with concomitant hypersplenism, autoimmune complications refractory to steroid therapy, and portal hypertension. The spleen becomes a site of extramedullary hemopoesis in myelofibrosis with myeloid hyperplasia, witch points in certain cases to the need of splenectomy.
