The purpose of work: to determine the most characteristic changes in the health of children exposed to of the Chernobyl catastrophe and the mechanisms of their origin in the remote periods of the Chernobyl catastrophe, to optimize the system of dispensary observation, to develop and to improve the methods of treatment and prophylaxis of the most widely distributed form of somatic pathology. Methods of researches and novelty: clinical, laboratory, instrumental, mathematical. Results and novelty: It was established, that in the remote times of postaccidental periods, children of radiation risk groups have a low health level that is confirmed by the index of pathological affection fluctuating within in the limits from 5,43 to 5,97. Chronic bronchorespiratory pathology was more often formed in children of ІІБ subgroup. The basis is the presence of bronchial hyperreactivity, disorders in the function of surfactant pulmonary system, the intensification of free radical processes and the decrease in factors of local defense of respiratory organs. The parameters of an operative regulation of cardiac rhythm specify the presence of vegetative disbalance, that is confirmed by high frequency of pathological variants of vegetative maintenance of activity with unsufficient activation (quick emaciation) of sympatho-adrenal system. The leader is chronic gastroduodenitis with a prevail of superficial inflammation in the structure of diseases of digestive system. The pathology of digestive system has as a rule combined character with involvement of several zones of the digestive canal in the pathological process. The functional state of the immune system is characterized by disbalance of immunoregulatory subpopulations, moderate suppression of cellular link and high percentage of combined dysimmunoglobulinemias, that form mainly the undifferentiated depressive type of immunological disorders in children of ІБ subgroup, and the depressive type - in the children of ІІБ subgroup. The average parameters of physical development are correspondent with age regional norms, but its disharmony is noted in children of ІІБ subgroup. General mechanism, causing the disorder of functional organs and systems and resulting in forming chronic somatic pathology in children living in the contaminated territories, is the activation of subradical processes in organism. In children which were born in the liquidators of the Chernobyl accident the phenomenon of genetic instability is formed that results in high risk of multifactor pathology realization. The regulation disturbance as alternative asthenia (stress of adaptation processes) and alternative depression (the emaciation of compensatory adaptation mechanisms) are the general mechanisms of forming the vegetative disfunction with arteriac hypotension in children of the groups observed. The schemes of dispensary observation of group children with high radiation risk have considerable significance for unificating diagnosis and treatment that will be contribute to the recovery and preservation of health.
