Aim of the work - to base the approaches to diagnosis of the pathological states and risk groups formation of their appearance among children suffered as a result of the accident at the Chоrnobyl APS on the basis of clinical experimental data of morphological, citochemical, biochemical and immunological signs of the secondary mitochondrial insufficiensy. Methods and apparatus: radiobiological, citochemical, morphological, microscopical, biochemical, instrumental, statistical. Results and their novelty. The generalization of the results of experimental researches on the cellular culture and the laboratory animals under the influence of various doses of radiation and regimes of an irradiation had shown, that one of the basic manifestations of secondary mitochondrial insufficiency can be the changes with different direction in the parameters of enzymatic activity of the key enzymeof the Krebs cycle - succinate dehydrogenase and alpha-glycerophosphate dehydrogenase, reduction of power-formation in mitochondria (phosphorylation oxidation), morphological and ultrastructural changes in mitochondria, a level of apoptosis, that confirms the results of the clinical researches. At ultrastructural, subcellular, cellular and systemic levels the presence of disorders of cellular bio-energetics which at a clinical level is manifested as 4 basic symptoms of complexes: astheno-neurotic, astheno-vegetative, cardio-muscular and immune deficiency syndrome was revealed in children, the inhabitants of the radioactive polluted territories. The algorithm for diagnosis of secondary mitochondrial dysfunction and the test of selective screening for formatting the risk groups of the development of secondary mitochondrial insufficiency was offered in children living at the radioactive polluted territories.
