Obiective: prevention of HSP development in boys. Methods: clinical, rentgenologic, biochemical, radioimmunologic, genetic, mathematical. Theoretical and practical results: There has been determined the informational value of anamnestic, clinical, metabolic, hormonal and genetic findings concerning HSP course in boys. The most informative of those were: burdened heredity as regards hypertonic disease, type II diabetes mellitus, obesity degree, BP, presence of dermal stria, character of sexual development, HOMA insulinoresistance index, melatonin, testosterone, cortisol, trigleceride and antigens (A11 and B16) of HLA system. There have been established prognostically favourable or unfavourable factors, and tables for formulation of individual prognosis for patients of different age have been worked out. There has been proposed treatment-and-diagnostic algorithm which provides for a differentiated choice of HSP therapy with consideration of individual course of the disease (favorable or unfavorable) and mechanisms of its development. It has been established in case of a favourable HSP course prognosis employment of conventional therapy is sufficient, provided repeated treatment course (2-3 times a year) is carried out. In case of an unfavourable prognosis the conventional therapy must be accompanied by employment of medications (metformin, ?-lipoid acid drugs and methionin) aimed at a decrease in insulinoresistance, dyslipoproteinemia, and activity of melatonin production. For a stable positive effect of HSP treatment under an unfavourable prognosis of its course it is necessary to carry out a clinical and laboratory examination every 3 mouths to determine a further treatment factics. Novelty: lies in workind out a technology for the treatment of patients with different HSP course taking into consideration mechanisms of its development.Effectiveness: The developed treatment technology for boys with HSP makes it possible to prevent development of the disease in 78.6-85.7% of cases.
