Sci Ukraine popup.description2 Objective: to clarify possibility of the prognosis of risk of development of cardiac arrthymia (ventricular extrasystoles of high gradation, often supraventricular extrasystoles, atrium fibrillation), of the elderly patients with IHD.Observation: During the first stage of observations 486 persons in the age of 60-79 years were examined.   At the second stage (results of 3 years observations) 417 persons in the age of 60-79 years were examined. 156 patients with IHD were examined for revealing predictores of cardiac death. The period of observation - 5 years. 40 people of which during observation have died. The reason of death - acute heart attack.Methods of a research: ECG of high resolution, surface ECG in 12 assignments with computing a dispersion of parameters, daily monitoring of ECG, daily monitoring of RR, echocardiography.The results of 3 years observations of the patients with IHD have shown, that independent predictors of the development ventricular extrasystoles of high gradation are represented by dQRS; availability VLP; LF; time of daily ischaemia. Independent predictores of development supraventricular extrasystoles are represented by time of daily ischaemia; a state of dyastolic function of the left ventricular; a size of the left atrium; a ejection fraction of left ventricular; dyastolic volume of left ventricular. The premises to development of the persistant form of the atrium fibrillation may be as follows: presence of frequent supraventricular extrasystoles; sizes of left atrium; presence of ALP, a state of dyastolic function of the left ventricular. The results of 5-years long observation of the patients with IHD in the age of 60 and higher have shown, that independent predictores of cardiac death are as follows: time of daily ischaemia, state of microcirculation; a level of spontaneous aggregation of platelets, index of elasticity of erhytrocytes; a LHD level; an ejection fraction of left ventricular.
