Purpose: To improve combination therapy of non-small-cell lung cancer by means of developing the techniques of using radiomodification with anti-tumor drugs based on investigation of chemoradiation influence on apoptosis induction in the cells. Methods of research: Biochemical, biophysical, electron microscopy, morphology, clinical instrumental, statistical. Results and their novelty: The efficacy of chemoradiation therapy with Taxotere, etoposide, cisplatin in patients with non-small-cell 2B-3B lung cancer was proven. The findings of clinical instrumental investigations (x-ray and CT) allowed establishing that immediate results and duration of relapse-free period are 20-30% better when compared with irradiation without chemomodification. At chemoradiation therapy with modification, ceramide (apoptosis inductor) accumulates in the tumor and blood cells. Various mechanisms of influence of radiation therapy on the amount of ceramides with different modificators were determined. To improve the efficacy, recommendations as to chemoradiation therapy in patients with lung cancer were proposed. Experimental investigations of Guerin’s tumor demonstrated that simultaneous action of ionized radiation and chemotherapy promoted induction of ceramide pathway of apoptosis and is important to overcome the tumor radioresistance. For the first time, it was shown that the mechanisms of influence on the tumor are different, namely: radiotherapy with Taxotere or etoposide increased ceramide synthesis de novo, together with cisplatin caused degradation of sphingomyelin and accumulation and ceramide accumulation while simultaneous action of chemo- and radiotherapy on the organism of the experimental animals promotes accumulation of the product of ceramide degradation (sphingosine) which possesses pro-apoptosis properties and inhibits synthesis of pro-proliferative sphingolipid, glucosyl ceramide.
