Obiective: determination of dysthyreosis impact on DM course and development of diabetic complications by more accurate defining the mechanisms of thyroid regulation disorders in children with DM. Methods: clinical, biochemical, radioimmune, genetic, instrumental, and mathematical. Theoretical and practical results: it has been established that in 66,7 % of children and adolescents with DM there are revealed certain signs of dysthyreosis, the most freguent variants of which are TTH content in crease against the background of TH normal findings, and isolated f T4 level increase. In 64,5 % of the examined patients, irrespective of changes in the levels of peripheral TH, there has been revealed a significant decrease in the reverse T3 content which makes it possible to exclude the presence of the so called “nonthyroid pathology syndrome” in them. It has been established that in childhood dysthyreosis formation against the backgroundof DM is first of all influenced by carbohydrate metabolism decompensation, as well as an increased level of contrainsular hormones, cytokines, and a decrease in melatonin-producing activity and the liver functional disorders. A negative impact of dysthyreosis in children and adolescents with DM on microcirculation disorders and psychic disadaptation development has been observed. A diagnostic model of the thyroid gland functional disorders in the presence of DM has been worked out which made it possible to develop an algorithm for thyreopathies diagnosing and therapy selection for children and adolescents. Novelty: there has been determined the pathogenetic mechanisms of thyroid function regulation in patients with DM, their difference from the “nonthyroid pathology syndrome” has been established, dysthyreosis influence on diabetic complications formation has been shown, and approaches to diagnosing and treatment of the thyroid gland pathology in children and adolescents with DM have been substantiated. Effectiveness: optimization of diagnosis and treatment of thyreopathies against the background of DM makes it possible to improve the technology of DM and its complications treatment in childhood and puberty.
