Objective: improving OA diagnosis in adolescents on the basis of determining early diagnostic criteria and the mechanisms of the disease development.Methods: clinical, roentgenologic, ultrasonic, biochemical, radioimmunological, immunofermental, and genetic. Theoretic and practical results: OA in adolescents, which has developed against the background of HMS, as well as after ReA, is characterized by frequent damaging of genual joints, though without disorders in their functions, and by minimal clinical manifestation, X-ray and ultrasonic findings. There has been established the significance of hereditary load in the development of OA, especially along the maternal line, and a high incidence of chromosomal aberrations has been revealed. It has been proved that during OA development in adolescents thereare observed disorders in collagen metabolism and proteoglycane insufficiency, accompanied by activated ferments of basic proteins catabolism in the connective tissue with a decrease in their inhibitors. Some data on inflammation participation in OA development have been obtained. There have been established some deviations in the microcirculation system whose incidence increases with the progress of the disease. Gender differences have been established in manifestations and in mechanisms of OA development. There have been worked out criteria for the disease early diagnosing, and a treatment course has been substantiated. Novelty: It has been established that during OA development in adolescents there take place disorders in metabolism of structural components of connective tissue with development of proteoglycane insufficiency and an expressed oxyprolinuria against the background of proteolythic ferments increase and their inhibitors decrease, activation of cellular and monocitic-phagocitic links of immunity,and the inflammatory process development. These processes occur under the conditions of high hereditary load and citogenetic changes. Informative diagnostic and differential criteria have been worked out for adolescents with OA which has developed against the background of ReA and HMS. Effectiveness: the use of said diagnostic criteria will make it possible to established initial OA signs in adolescents with articular pathology and to define an appropriate course for treatment and prophylaxis.
