Malaya of Ukrainian Academy of Medical Sciences" popup.description2 168 patients with stable angina of effort of II and III functional class, 187 patients with unstable angina (progressive angina of effort) and 10 healthy volunteers were testified by instrumental (test with reactive hyperemia, treadmill-test) and biochemical (spectrophotometric, enzymatic and immuno-enzyme assay) methods. It was established, that dislipidemia results in considerable violation of platelets morpho-functional and aggregative properties, that was instrumental in the rise of platelets hemostasis activity and aspirin resistance development, at what the most essential reduction of aspirin antiaggregative effect take place at II B type dislipidemia. It was elucidated, that at destabilization of ischemic heart disease flow, quantity of patients which had the lowered answer for the aspirin therapy increases. The differentiated approach to setting of antiplatelet therapy depending on an initial aspirin sensitivity, mechanism of platelets hemostasis activation and presence of lipid metabolism distruction at patients with different variants of ischemic heart diseases flow was grounded. It was proved, that therapy with the use of simvastatin results in platelets hemostasis activity reduction both due to lipid spectrum normalization and due to the rise of sensitivity to antiplatelet therapy. The expedience of aggressive approach to setting of antiplatelet preparation at patients with both ischemic heart disease and lipid metabolism distruction was grounded. Efficiency of antiplatelet therapy at patients with ischemic heart disease during 1 year was appraised.
