Objective: improvement of therapeutic and sanitary programs for adolescents with PAH, on the basis of studying dynamics of changes in target organs (heart, cerebral vessels and kidneys) in the process of catamnestic observation. Methods: clinical, resting ECG, ECG with physical load, echocardiography, Doppler-echocardiography, Doppler of renal vessels, BP and HR daily monitoring, rheoencephalography, blood lipid spectrum, levels of E and NE urinary excretion, plasma rennin activity, aldosteron and angiotensin levels, and microalbuminuria. Theoretical and practical results: in adolescents with non-adequate BP profiles at night (non-dipper, over-dipper) against the background of some characteristics of sympathico-adrenal system (norepinephrine) and renin-angiotensin-aldosteron system (angiotensin II, aldosteron) gettingactivated, there takes place formation of desadaptive LV myocardial hypertension, endothelial dysfunction of renal glomerular apparatus (microalbuminuria), and of atherogenic balance of blood lipid spectrum. There were singled out prognostic criteria of PAH unfavorable course in adolescents and in young persons (aged 25-30), in which AH had formed in puberty. An algorithm for prognostication of PAH unfavorable course in adolescents was worked out, and certain approaches to rehabilitation measures were defined with consideration of the singled out criteria. Novelty: it was proved that in adolescents with PAH BP pathological profiles were accompanied by disorders in target organs (heart, cerebral vessels and kidneys) and by formation of atherogenic dyslipoproteidemia. There were singled out some criteria, and an algorithm for prognostication of PAH unfavorable course in adolescents was worked out which made it possible to develop a technology "Prognostication of renal disorders in adolescents with PAH". The purpose for therapeutic and prophylactic measures was determined. Effectiveness: employment of said algorithm makes it possible to single out, at early stages of the disease, persons which need administration of differentiated complexes of antihypertonic therapy taking into consideration types of BP daily monitoring and their neurohumoral background.
