Object of investigation: development of hypoxical myocardial remodeling in rats and human; its molecular and genetical mechanisms. Aim of work: to investigate molecular and genetical mechanisms of protective phenotypical restructurization of myocard under high altitude hypoxia influence. Methods of investigations: molecular biological (RT-PCR, immunoblotting), morphological, biochemical, cytological, functional. The elaborated investigations let to characterize mechanisms and age-related peculiarities of myocardial remodeling development under high altitude hypoxia. It was established that mechanisms of favorable development of phenotypic remodeling involve transcriptional regulation of HIF-1alpha gene, increasing of expression of HIF-1alpha inhibitory factors: HIF-3alpha, iNOS and its stimulators HSP70 family proteins; intensifying production of tissue cytoprotectors - NO, chaperones, granulocytic colony stimulatory factor - during acute period after hypoxia influence, erythropoetin - during incomplete adaptation period. For a first time the peculiarities of expression of these genes and proteins in right or left ventricles of heart as well as in lung were found; the participation of hypoxical injury of tissues and host defence system activation in the hypoxic remodeling initiation, and transcriptionally mediated changes in tissue response to hypoxia during process of adaptation were characterized. The results obtained let carry out recommendations for using of selected hypoxia regimen for controlled development of adaptation and myocardial remodeling under high altitude or plane conditions; give perspectives for clinical investigations.
