Methods: clinical, instrumental, biochemical. Statistically, development of hypertrophic type of heart remodeling, thickening of intima-media of total carotid arteries and manifestations of diastolic dysfunction of left ventricle are more frequently observed in patients with essential hypertension in combination with diabetes mellitus type 2 than in patients with essential hypertension and normal hydrocarbon exchange. It is found that in this type of patients, the processes of cardio-vascular remodeling are influenced not only by hemodynamic loading and metabolic changes but also by an increases of synthesis of insulin-like growth factor-1 (IGF-1) and transforming growth factor-В1 (TGF-В1). It is determined that the most unfavorable type of heart remodeling-concentric hypertrophia of left ventricle is characterized by a high level of malonic dialdegide, IGF-1 and TGF-В1, microalbuminuria in blood, while eccentric hypertrophia of left ventricle is accompanied by a noticeable decrease of IGF-1 levels. An effective adaptive therapy scheme is developed, which takes into account initial features of cardiovascular remodeling and levels of growth factors and uses perindopril with amlodipin or nebivolol.
