Results: in DN the progressive rising of the РАІ-1 level is determined, that plays a potential role in the developments of a glomerulosclerosis and interstitial fibrosis. The accelerated glycosilation end products and angiotensin ІІ stimulate secretion of РАІ-1 by blood monocytes of the patients with DN. One of mechanisms of positive influence on kidneys of angiotensin blocking drugs is normalization of balance between components of fibrinolytic system - urokinase and PAI-1. The revealed rising of РАІ-1 concentration may help in definition of well-timed and proved therapeutic tactics on incipient states of disease. Between the contents in plasma РАІ-1 and nitrites there is a close revertive correlation. The ascending in a blood of the contents of a nitrite under influence of therapy and decrease of the total NO first of all is caused by restriction of activity of a link ІNOS.
