Meaningful piculianties of the violations of endothelium destruction enzymes activity and proteinas inhibitors were revealed at COPD patients and their role in the processes of cardio-vascular resimulation was determined. At was proved that COPD development results in the system of proteinaze inhibitor at the expense of the activity reduction of the elastaz of different origin and increasing elastazoinhibitoric activity of a-1-inhibitors of proteinaze (a-1-IP) that is stihulated by the development of inflammation processes, destructive tissues transformations and may lead to the loss of muscular mass and force, worsening of the outer breathing function and hypoxemia. It was proved that COPD is characterize by considerable reduction of the activity of endothelium elastaze and insignificant increase of elastazoinhibitoric activity a-1-IP. The activity reduction of endothelium elastaze at COPD patients is connected probably with endothelium injury in bronchial-pulmonary system and endothelium dysfunctions development. Early predictors of chronic pulmonary heart based on the indications of the endothelium destruction enzymes activity, forecasts concerning criteria development and strengthening o hemodynamic and ventilation infringements at COPD as well as forecasting of chronic pulmonary heart are developed. Criteria of the application of new COPD therapy schemes with the inclusion of the methods of endothelium dysfunction correction (nebivolol+epadol, phenspirid+nebivolol, phenspirid+ epadol) depending upon the state of endothelium dysfunction.
