Subjects: 15 healthy young (20-34 years) and 18 healthy elderly (60-74 years) subjects, and 87 elderly (60-74 years) and 12 young patients (20-34 years) with duodenal ulcer disease. Methods: fibroesophagogastroduodenoscopy, basal topographic pH-metry, 24-hour intragastric pH monitoring, histological study of gastric mucosa bioptates, gastric secretion fraction study, pentagastrin test, spectral analysis of heart rhythm variability (HRV) . Results: Changes of circadian rhythms of intragastral pH in elderly healthy subjects indicated decrease of gastric acid formation under physiological aging. Compared to healthy subjects, the elderly patients with DUD displayed disturbed circadian rhythms of intragastral pH showing no essential pH fluctuations during 24 hours, particularly at night. This finding pointsto the higher acid production level. Compared to young healthy and DUD patients, their elderly counterparts had a decreased activity of the parasympathetic link of autonomic nervous system and no essential rise of the parasympathetic tone occurred at night.This evidences for a lesser role of parasympathetic nervous system in DUD pathogenesis in the elderly.
