Due to intensive conservative therapy it was succeeded to obtain the "precipice" of acute pancreatitis development on the stage of edema form in 73 (89,02%) of patients. Aseptic pancreonekrosis is initially exposed in 45 (27,4%) patients. In them diagnostic laparoscopy is conversed in curable. The exposed liquid accumulations punctured under the control of ultrasonic research. Septic pancreonecrosis developed in 27 (16,4%) of patients. At ineffective miniinvasive "half-open" interventions patients are operated under the traditional "opened" method. Development of purulent complications of aseptic pancreonecrosis was succeeded to prevent in 47 (87,0%) of patients. The analysis of medical treatment of 344 patients with the focal defeats of liver is conducted. There are 179 patients with a colorectal cancer with metastases in liver, 69 patients with the non - vermin cysts of liver and 96 patients with echinococcosis. Cryodestruction is applied to 197 patients, from them in 119 patients with a colorectal cancer, 34 patients with non- vermin cysts and 44 with echinococcosis. The positive effect of cryodestruction is marked, that is confirmed by reduction of number of complications in the nearest and remote period, diminishing of bed- time, reduction of lethality. In 61 patient PDR was executed concerning the cancer of the head of pancreas(29), cancer of periampular area (20) and major pancreatic duct (12). In 10 patients with cancer of the head of pancreas and periampular area, complicated by single metastases in a liver, PDR is complemented by cryodestruction of metastases. It is set, that cryodestruction of pancreatic stump in the process of the PDR implementation allows to decline the frequency of complications in early posteoperative period, and also diminishes the risk of intensification of chronic pancreatitis in a remote period. Cryodestruction of metastases of liver prolongs the median of life of patients.
