OBJECTIVE: to reveal the pathogenic peculiaritis of inflammatory diseases of sphenoidal sinus, to elevate diagnostic authenticity and to prevent genesis of intracranial and orbital complications. RESULTS: the anatomical variants of sphenoidal structure and skull base adjacent structures have been learned. The estimating criteria for computer-ultrasonic section's data in patients with chronical sinusitis have been worked out. That criteria admits timely to bring up different abnormal variants of nasal and paranasal sinuses structures. That criteria admits to produce optimal surgical approach and to avoid intraoperational complications. The optimal order of implementation of endoscopic surgical aggression has been designed for polysinusitis in combination with different changes in intranasal structures. NOVELTY: for the first time the technique of endoscopic surgical procedures has been submitted for sphenoiditis in patients with polysinusitis in combination with different changes in intranasal structures. The original apparatus for sparing correction of middle nasal concha's fofm and capacity has been worked out. EFFICIENCY: acceleration of medical period for patients with sphenoiditis frequency, reduction of intracranial and orbital complications.
