Aim of the work: To improve the efficacy of treatment for patients having acute pyelonephritis (APN) and the accompanying inflammatory diseases of genitals and thyroid gland by means of improving the etiologic diagnostics of the inflammatory processes, disclosing the leading links of disordered local immunity and defining the leading links of the disordered local immunity and defining the basic mechanisms of etiopathogenesis of ureaplasmic infection in modeling pyelonephritis on animals. Methods of investigation: microbiologic, immunologic, cytomorphologic, moleculo-genetic, creation of models of pyelonephritis and endometritis on animals, biochemical, clinico-instrumental, statistical. Apparatuses: amplifier, Stat Fax 2100 immuno-enzymic analyzer, Karl Zeiss axioscop 40 light microscope, Ultimapa doppler ultrasonic device, Мефан 8001 analytic photometer. The patients with APN and aggravation of chronic demonstrated the infectious pathogens in 61.8% of cases: in monoculture the bacteria were 8.3%, molicutes - 34.7%, C.trachomatis - 4.2%; mixed infection was found in 16.0% of patients. The infection caused by molicutes in 40 - 45% was accompanied by the development of specific antibody response. It has been proved that the source of infection (molicutes) in the APN patients is genitals. We have studied in detail the state of humoral and cellular links of local immunity in APN, disclosed the immune mechanisms of infectious inflammatory process on the local level. There were defined the peculiarities of clinical picture in APN patients in the presence of pathogens of various taxonomic origin which was accompanied by impairment of not only kidneys but also genitals, heart and thyroid gland.There was established the development of autoimmunic reactions, connected with inflammatory processes stipulated by, first of all, molicutes and cytomegalovirus. 10% of APN patients, with the help of dopplerography of kidneys, had limited hemodynamic reserve of this organ. While modeling pyelonephritis and endometritis on the laboratory enimals, there were firstly defined the mechanisms of pathogenesis of ureaplasmic infection. It was stated that, in spite of the way ureaplasma was introduced, the latter disseminate with the blood flow. It was clinically proved that when in APN patients the mixed infections of molicutes and bacteria were found out the prognosis of a disease recurrence were the highest. There were determined the ways for improvement of treatment for APN patients with the help of additional prescription of etiotropic therapy in relation to molicutes and usage of probiotic per os and intravaginally.
