Research methods. In-process the applied instrumental (ekhocardiography, luminescent and electronic microscopy), immunological and biochemical methods of research. The results of researches showed that in all groups of patients there are the considerable changes in a state of cardiac hemodynamics of all chambers of heart (the function of LV more suffers), changes of indexes lipids and protein oxidation, that are typical for oxidative stress and violation from the side of all links of immunity. It was found out the changes from the side of oxygen-depended metabolism of neutrophils and macrophags, cytokines (TNF ) and humoral link of immunity (increase of level of CІC and AB), level of СRP. The changes found out testify to the presence of immunopathologic reactions (autoimmuny, immune-inflammatoryand immunodepressive) in patients from DCM, COR and IHD groups. It was well-proven that at application of slow titration the overwhelming amount of patients is able to attain the high therapeutic doses of carvedilol (100 mg/daily) and enalapril (20 mg/daily). Long-term treatment with the use of such doses brings to the proof and considerable improvement of patients' FC and to reverse remodeling at part of patients from a group CM. In 30 % such patients there was normalization of cavity of left ventricle, and in 52 % - normalization of its EF. In the experiment discovered, that most the demonstrated displays of immuno-inflammatory defeat of heart at mice were detected on the sixth month after immunization. The changes which gain making progress character are given in retractive myocardium as pathological heterogeneity from different degrees adaptations and disintegrator phenomena, that were present in different proportions of different cages, result in their hypertrophy atrophy, and sometimes and to the irreversible damage. General motion of process answers the signs of reaction of hypersensitiveness of slow type, and mechanisms of damage of ventricular cardiomyocyte is certain as similar onkoziz, that takes place with bringing in of lysis-ubiquitin-shaperon-proteasomal process, original variant of autophagy with active voice of lysosomal cell apparatus and apoptosis mainly in his membrane-mitochondrial variant.
