Komisarenko Institute of Endocrinology and Metabolism оf the Academy of Medical Scienses of Ukraine" popup.description2 Object of observation: epidemiology of Diabetes Mellitus type 1 (T1DM) in Ukraine: mortality, chronic complications, and cardiovascular risk factors in regions with different T1DМ prevalence. Aim: epidemiologycal researh in groups of patients with the course of deseases typical of T1DM is the bases for prospective and cross-sectional studies of acute chronic complication in patients with diabetes of the type 1. Methods: observational analysis of risks (according to Hazard Ratios and Odds Ratios) of general and diabetes related mortality. Analysis of occurrence of proliferative retinopathy (PR), diabetic nephropathy (DN), arterial hypertension (AH) among T1DM patients. We have compared the levels of C-peptide, urine albumin (A), homocysteine (Hcy), glutamate decarboxylase antibodies (GADA), insulin antibodies (InsAb), genetic polymorphism of PPAR-gfamma receptor activator, as well as methylenetetrafolate reductase (MTFR) among patients from different T1D territorial clusters. Results: diabetes related mortality, as well as mortality related to acute diabetic complications for patients from maximal cluster (MAX) of T1DM prevalence comparing to the minimal (МІN) one is much higher. T1DM complications, AH, high HbA1c levels and persistence of GADA are much more frequent (P<0.01) in the MAX cluster, than in the МІN for PR, DN, AH, increase of HbA1c and GADA respectively. We have found a variations of genetic polymorphism or PPAR-gamma depending on T1DM prevalence. T1DM patients that live on territories with higher T1DM prevalence are more likely to develop chronic complications and to die from them, than patients, who live on territories with lower T1D prevalence. Novelity: anallysis of data base about treatment of T1DM with insulin using mathematic model operating with scope of indivsdual data and allowing to distinguish groups of patients with typical history of T1DM. The difference of the method proposed is that it allows to avoid technical mistakes, standartise data concerning desease duration, determine and compare average and individual daily doses of insulin with the help of linear regression, concludes of adecvate insulin treatment, determines groups of patients needing estimation of treatment efficiency.
