Objective. The aim of the study was to evaluate the accordance of medical care delivered to hypertensive patients in a primary health care setting - State Institution of Science "Research and Practical Center of Preventive and Clinical Medicine" State Administration Department - with Clinical Guidelines of the Ukrainian Association of Cardiology on Hypertension prevention and treatment (2008). Methods. Clinical investigation, routine laboratory assessment, including complete blood count and blood chemistry investigation, urinalysis, electrocardiogram, echocardiography, and medico-statistic analyze were performed. Results. The retrospective analyze of medical care delivery to hypertensive patients as registered in outpatient's medical cards has been made. It was found, that most patients with hypertension received medical care - clinical examination, investigation, prescribed antihypertensive treatment - as required by Clinical Guidelines of the Ukrainian Association of Cardiology on Hypertension prevention and treatment (2008). Blood pressure lowering drugs were prescribed in accordance with evidence-based hypertension management recommendations - 96,8% of patients received first-line antihypertensive drugs, 68,6% - combination therapy. The percentage of achieved target blood pressure level as reflected in outpatient's medical cards was higher than in USA, EU and the mean level achieved in Ukraine - 61,7% vs 34%, 33,6% and 18,7%, respectively. The most common breakdown of ambulatory medical cards was failure of data on cardiovascular risk stratification. Data on symptoms, medical history, initial assessment and follow-up were found in all outpatient's medical cards. Data on risk factor stratification, risk-lowering intervention and lifestyle modification generally were missing or difficult to found. Our future strategy for successful guideline's implementation in ambulatory setting will be to develop a methodology plan including how the data on risk factor stratification, risk-lowering intervention and lifestyle modification will be collected at baseline and after intervention. Implementation of standarts medical records flow-charts and checklists should be the part of our future workup. We have also to advance the level of knowledge of health-care professinal's in the area of hypertension prevention and treatment and promote patient's education with information booklets. We established the need for development and implementation of standards medical chart on cardiovascular risk stratification and risk-lowering intervention.Field of application: activities of medical institutions.
