Gorky Donetsk National Medical University popup.description2 During investigation of 1118 patients with essential hypertension (EH) and 205 patients with EH and concomitant osteoarthritis (OA) it has been determined that the most severe clinical course, disorders of the daily rhythm of blood pressure (BP), systemic and renal hemodynamic, endothelial function, water and electrolite balance, renin-angiotensin-aldosterone (RAAS) and sympathetic nerves systems (SNS) activity, their reactions to different salt rations, cardiovascular risk are features of saltsensitive EH as compared with saltresistant and paradoxical types. Prognostic biomarkers of developing saltsensitive EH are paradoxical EH reactivity, tend to tubular-glomerular disbalance, microalbuminuria, hyperfiltration in high salt fase and RAAS hyporeactivity in salt load. Nebivolol, losartan, felodipine are preferable in the saltsensitive patients. In the patients with EH and concomitant OA correction of calcium and magnesium metabolism disorders improves the daily rhythm of BP, dcreases osteoporosis, delays bone remodeling and reduces signs of OA.
