Aim of the study - determination of total cardiovascular risk on the base of factors stratification which influence on formation and course of cardiovascular diseases jin patients with metabolic syndrome that allow to improve early diagnostics and optimize treatment of diseases associated with metabolic abnormalities in cardiovascular diseases. Methods of the study - clinical, anthropometric, laboratory, instrumental, statistical. Results and their novelty. Relationships between formation and development of dysmetabolic abnormalities (insulin resistance, hyperinsulinemia, hyperglycemia, dyslipidemia, abdominal obesity) and hypertrophic types of left ventricular remodeling were evaluated. In the patients with AH and metabolic abnormalities elevated levels of beta2-microglobulins, microalbuminuria, insignificant decreasing of glomerular filtration speed were detected that suggest early tubule-glomerular nephron affection. In patients with excessive body mass and obesity increased production of adipokines (hyperleptinemia, hypercytokinemia) and insulin resistance, which influence on summary cardiovascular in the patients with MS. On the base of complex study of clinical, anthropometric, echocardiographic, morphological data, parameters of cytokine and lipid blood profile relationship between formation of LVM remodeling, hyperactivation of TNF-alpha and IL-6, dyslipidemia were detected depend on presence, degree and type of obesity in AH. In patients with AH accompanied by obesity endothelial dysfunction was found, that manifested by vascular remodeling by elastic type, decreasing of dilation vascular response and increasing of vasoconstriction during probe with reactive hyperemia, dysbalance of vasoactive nitric oxide pool, endothelin-1 elevation in systemic circulation, inhibition of antioxidant system that resulted in destabilization of clinical AH manifestation in obesity progression, and it was found that nebivolol along with antihypertensive effect improve functional endothelium condition. Economic efficacy, significance of the study and conclusions - methodology of MS in patients with AH was developed and proposed that allow optimize and improve early diagnostic of metabolic abnormalities among population of Ukraine, to prognoses risk of cardiometabolic exacerbations and to define strategy of non medical and medical correction of MS components that will promote improvement of life quality, reduction of economic expenses on medical service.
