The purpose of the work - to assess the impact of chemoradiomodifiers and interferon on the efficiency of combined radiation therapy of patients with ACC by determining the molecular-genetic characteristics of tumor process aggressiveness, close and long-term results of treatment. Methods of examinations and instruments: clinical, laboratory, radioisotopic, dosimetric. Simulator of a radiation session - "SimView" 3000 (Siemens, Germany), TERATRON, AGAT-VU (MDR), US-unit "Aloca" - FLEXUS, device for magnetic resonance imaging procedure (MRI) "Magnetom-Vision-Plus-System" ( Siemens), universal dosimeter PTW Freiburg "Unidose E", Centrifuge "OPN-3" with revo 2000g, flow cytofluorimeter "FAScan" (BECTON DICKINSON, USA) № 82582, 1991. The main groups are 110 patients with ACC, who were underwent combined radiation therapy against application of chemoradiomodified remedies (capecitabin, ftorafur, gidrea) and laferon, and 39 patients of the control group, which were underwent combined radiation therapy without using of radiomodifiers. The indications for the use of developed chemoradiotherapy technologies of patients with ACC are determined, taking into account specific features of the neoplastic process. The possibility of using molecular-genetic parameters of ACC was established for the prediction of conservative treatment and evaluated its effectiveness. The dependence of the sensitivity of advanced forms of cervical cancer was proved to cytotoxical effects on indicators of proliferative activity of tumors, identified by DNA flow cytometry. It has been established according to the results of molecular genetic studies that combined radiation therapy of patients with ACC against application of chemoradiomodified remedies is more efficient. The obtained data correlate with the results of clinical and instrumental examination of patients with ACC. It was developed an algorithm for dynamic monitoring of combined radiation therapy for patients with ACC (a set of clinical, morphological, radiological and molecular genetic examination) in order to evaluate the intermediate and final objective response of ACC by cytostatic therapy. The techniques of combined radiation therapy against application of chemoradiomodified remedies improve the immediate results: objective tumor response (complete + incomplete involution) with using of capecitabin and interferon increased to 19%, ftorafur and interferon - to 20%, by using of gidrea in combination with interferon - at 15% in comparison with the control group, which has received only radiation therapy, what indicates about their effectiveness. Manifestations of general toxicity after combined radiation therapy of the main and control groups, which would exceed the grade II and would adversely affect the quality of patients' life, were not observed.
