Purpose of work - to promote efficiency of head and neck cancer patients' treatment by development and improvement of combined, extended and reconstructive plastic operations. Research methods: clinical, laboratorial, roentgenological, histological and statistical. Results: The new methods of reconstruction of head and neck defects after the combined and extended operations are developed and the existent methods are improved using dermic-fatty, musculocutaneous, free transplants and shreds on a vascular leg, which provide a high functional effect and improve quality of life of patients as compared to the traditional methods of the plastic. A new method of the plastic reconstruction in patients with total lower lip defects by the use of chin and subchin areas tissues is developed. Usage of combined, extended and reconstructive plastic operations with the use of both flaps on a feed-in leg and free transplants, provided high 2-year survival (55,5 ± 8,3 % and 61,4 ± 7,3 % accordingly) for patients with locally advanced head and neck cancer.
