Objective: To investigate sensitivity of blood serum sphingomyelinase in patients with non-small cell lung cancer before chemoradiation therapy and its role in production of apoptosis inductor, ceramide, with the purpose to improve the control and prognosis of the treatment efficacy. Methods: Biochemical, electron microscopy, morphology, clinical instrumental, radionuclide, statistical. Results and their novelty: Efficacy of chemoradiation therapy with etoposide in accelerated fractionation mode when compared with both traditional and accelerated irradiation without etoposide in non-small-cell lung cancer was demonstrated. It was proven that activity of acid secreted sphingomyelinase increased 2.6 times, ceramide amount in 86.2% of patients after chemoradiation therapy with etoposide against a background of the tumor regression over 50%. For the first time, the possibility of application of active acid sphingomyelinase during chemoradiation therapy with etoposide was considered. The criteria of application of the ratio of activity of acid sphingomyelinase before to after treatment were worked out. The experiments allowed establishing that combined action of ionizing radiation and etoposide promoted increase of sphingomyelinase activity in the blood serum against a background of apoptosis process activation in the tumor. Efficacy of introduction: 5 articles, 6 abstracts were published. 2 patients for a useful model were obtained. Sphere of application: Radiation medicine, oncology.
