The aim: to determine peculiarities of insulin resistance (IR) development and impaired glucose tolerance (IGT) during aging. Population: healthy people 20-39 (40 sub.), 40-59 (38 sub.) and 60-79 years (65 sub.). Methods: anthropometric, biochemical methods of determining of glucose and lipids in the blood, radioimmunoassay determination of insulin, C-peptide, cortisol, leptin, growth hormone levels in plasma, a standard oral glucose tolerance test, laser Doppler flowmetry, bicycle ergometry. Devices: glucometer, laser Doppler flowmeter, television capillaroscop, electrocardiograph, radioimmunoassay kits. Results: the detection rate of insulin resistance (IR) and impaired glucose tolerance (IGT) are increase with age, In elderly people IGT developed as tissue IR with relatively preserved function of insular apparatus, or as a result of functional deficiency of the pancreas beta cells if IR absence. The frequency of visceral and general obsity which are factors in the development of IR and IGT, increases with age. Elderly people with IR have elevated plasma C-peptide level, which indicates an increase in the synthesis of insulin from pro-insulin. Blood plasma leptin concentration increased with age in women with symptoms of visceral obesity, especially when combined IR and IGT. Elderly subjects with IR decreases physical performance and efficiency of the functioning of the cardiovascular system under exercise, increases blood levels of cholesterol, triglycerides, LDL and VLDL, deteriorating endothelial function of microvessels. Novelty: the age-related peculiarities of the IR and impaired glucose tolerance with aging, developed a method for diagnosis of early disturbances of functional reserves of the pancreas beta cells in the elderly.
