Malaya of Ukrainian Academy of Medical Sciences" popup.description2 Object of research - 260 patients with ischemic heart disease (IHD), from them 58 with an unstable angina pectoris (UAP) and 202 - with a stable angina pectoris (SAP) of II-III functional classes. The used methods were: clinico-tool (an electrocardiography, echocardioscopy, rentgentelevision), clinico-laboratory and biochemical (conductometric, spectrophotometric, immunoenzymatic and enzymatic, turbidimetric). The used equipments were: hematological and immunoenzymatic evaluators, agregometer, photometers. It was demonstrated that hemovascular disturbances at an exacerbation of IHD flow are characterised by both changes of platelets morfo-functional properties, and ascending of proportion of patients which have reduced answer to aspirin therapy. The interrelation of immunoinflammation and oxidative stress parameters with hemostasis activity, and also their influence on sensitivity to aspirin was taped. It was proved that at UAP oxidative stress activity ascending may be a prognostic marker for aspirin resistance development. It was stated that significant disturbances of an endothelium functional state at stable disease flow are more characteristic for aspirin resistant patients. It was proved that lipid exchange disturbances at patients with IHD promote aspirin resistance development at what the greatest changes descended at dyslipidemia of IIB type. Means of revealing of atherothrombotic high risk patients and approaches to antiplatelet therapy appointment taking into account the mechanism of platelets hemostasis activation, initial sensitivity to aspirin and presence of lipid metabolism disturbances at patients with different variants of an ischemic heart disease flow were developed.
