Aim of investigation - it was showed the peculiarities of epileptogenesis and the pharmacoresistant somatic focal epilepsy at the fire brain damage of different genesis for pathogenic reasons of indices to surgical treatment and improvement the technology of open surgical intervention under such forms of epilepsy. Methods of investigation. Methods of investigation - clinico - anamnestic, clinico - neurologic, electro - neurologic, electrophysiologic surgical intervention, immunologic ,histologic and histochemic methods. Reults of investigation - it was revealed the existence in perifocal zone the fire brain damage of different genesis of destructive - reparative process which is helping to form in this zone the fire of epileptic activity and the pharmacoresistance to antiparoxysmal drugs.
