The purpose of the investigation is to improve the level of medical care for men and women who suffer from infertility in marriage by developing an algorithm diagnosis of disorders of functional state of the pituitary and gonads and to base the approaches to therapy. Methods of investigation are clinical, biochemical, immunoassay, cytological, chemiluminescent, statistic. The clinical laboratory investigations of men were held: 150 patients with idiopathic pathospermia, 20 infertile patients with hypothyroidism and 32 with semen parameters, corresponding to the parameters of World Health Organization. The signs of overt or covert androgen deficiency were revealed in men with idiopathic pathospermia. The use of therapy, aimed at androgenization increase of an organism leads to significant increase in the frequency of normalization of semen parameters while the testosterone level in blood is less than 12,0 nmol/l. The L-thyroxin treatment promotes significant improvement of spermatogenic and endocrine functions of testicles in infertile men with hypothyroidism. The examination of 240 women with infertility in marriage has shown that hyperinsulinemia in women with polycystic ovaries may be as well in patients with obesity and normal weight. The violation of the circadian rhythm of melatonin is observed in women with polycystic ovaries. The use of metformin in combination with estrogen-progestin medication promotes the decrease of ovarian androgens level, normalizes the carbohydrate metabolism disorders and increases the efficiency of conservative therapy under the stimulation of ovulation in women with polycystic ovaries. In women with hyperandrogenism the number of oocytes obtained by aspiration from follicles and the number of embryos of high morphological quality formed of them is significantly lower than in patients with normandrogenism. The pathogenetic role of blocking of phosphodiesterase activity in the prenatal period in the development of hypofunction of sexual glands in male rats was determined; the positive effect on testicular function of drugs tribestan, chondroitin sulfate and bioglobin was revealed.
