Danilevsky Institute of Endocrine Pathology Problems of Academy of Medical Sciences of Ukraine" popup.description2 The objects of the investigation are: polymorphism of adiponectin gene, the level of this hormone in the circulation, the formation of insulin resistance for patients with type 2 diabetes mellitus (T2D) after the account of presence and absence of metabolic syndrome. The purpose of the work is to determine the character of single nucleotide polymorphism (SNP) of adiponectin gene and its association with the hormone level in the circulation, as well as the contribution into the formation of insulin resistance; to define classic risk factors of atherogenesis, improvement of algorithm of rehabilitation therapy of T2D, and its vascular complications after the account of patient's genotype features. Methods of investigation: biochemical, instrumental, population-genetic, molecular-genetic, statistic analysis. In the patients examined on T2D with predominance of glicemic decompensation the expressive signs of insulin resistance and oxidative stress were revealed. The differential contribution of genetic components was determined in dependence on locus of single nucleotide polymorphism of adiponectin gene (SNP +276G>Т, +45Т>G ADIPOQ) and present genotype in forming of diabetic phenotype of insulin resistance and in the sensitiveness to insulin on determination of the modulating influence of SNP gene of ADIPOQ on the level of circulatory adiponectin. Determinative value of metabolic disregulation (glicemic- and lipotoxity) for genesis of hyipoadiponectinemia was proved. In patients with T2D the preservation of determinative role of Q192R PON1 type for the genotype in the pattern of substrate dependent (diazoxonase/paraoxonase) of hydrolytic activity of antioxidative enzyme of paraoxonase was verified. In the use of molecular-genetic and population-statistical analysis was made the conclusion that homozyhosity on each of probed single nucleotide polymorphysm (+276G>T and +45Т>G gene of ADIPOQ, Q192R of gene of PON1) promotes the authenticity of T2D development, and heterozygosity forms the increased resistance to the disease.
