Aim: To assess prevalence and morbidity of the most severe disabling neuro-psychic disorders among the Chernobyl clean-up workers based on survivors' registries. Methods: clinical, epidemiological, medical-and-statistical. Results: According to cohort epidemiological study on the data of SRU (68145 men with whole-body external doses) and CER (8214 persons, including 3700 ones with whole-body external doses), the dynamics of psychoneurological morbidity in the Chernobyl clean-up workers (men) 1986-1987 was identified. Within 1988-2008, an increase in morbidity (encephalitis, myelitis, encephalomyelitis, stroke, cerebral infarction) was noted, it was especially pronounced in the last stage of observation (2003-2008), morbidity in the older age group exceeded that of persons under 40 at the time of exposure. Registry of disabling neuro-psychic disorders (about 1000 men at this stage) is under development on the data of SRU and CER. The results of random clinical examination of those enrolled in the registry were used to form the local subregistry (267 persons), to determine their neurologic and psychosomatic status, to reveal disease risk factors. Cerebral atherosclerosis, hypertension, and their combination were the main causes of acute stroke. Comorbidity pathology (organic personality disorders, vascular dementia, chronic cerebrovascular and somatic pathologies) was diagnosed in examined clean-up workers of 1986-1987 with depressive disorders. Post-traumatic stress disorder in clean-up workers was characterized by neuropsychiatric comorbidity, limbic system dysfunction with the possible hippocampal lesions on the background of cerebrovascular pathology.
