Aim of study: the elucidation of immunity state in preclinical latent period of type 1 and type 2 diabetes mellitus appearance (DM1, DM2). Methods: hematologic, cytologic (electronic microscopy) and immunologic. Theoretic and practical results: screening of normoglycemic children with aggravating heredity for a presence of increased titre of recently opened autoantbodies against the islands of Langerhans (IAA) in them and the possibility of their use as a test for prognosis of CD1 manifestation and studying mechanisms of human immune status change in latent period of CD1T development was conducted for the first time in the Ukraine. The high diagnostic value of determining IAA titre for prognosis of CD1T appearance in healthy persons in some regions of the Ukraine was shown by the performed investigations. Priority results: the data connected with a number of children with aggravating hereditary and positive to IAA and with very great probability of CDT1 appearance were obtained. CD1T was appeared in 48,5 % among children positive to IAA during 30,9±3,2 months, and in negative ones – in 8 %. It was noted that the significant changes in some immunity indices were revealed in healthy IAA positive children long before (sometimes some years) the clinical manifestation of CD1T: a decrease in the immunity level of subpopulations CD3+, CD4+ and CD56+ cells, and IL-4 level, and an increase of IL-1a, IL-6, TNFa and also chemokines IL-8 and IL-16, changes in which degree were more significant than in manifested diabetes. On this basis the risk groups that were significant for timely prescription of treatment and prevention of early complications were formed. The study of immunity status in preclinical period of CD2T was based on its changes in Metabolic syndrome (MS) that is a predictor of CD2T. The significant increase in IL-6 and TNFa levels was noted in 3 groups of patients (MS, DM2 and MS+DM2) under an examination of circulating adipocytokine level (IL-6, TNFa, leptin and adiponectin), while the leptin level was increased and that of adiponectin was decreased in MS. Increased leptin level in DM2 was only noted in simultaneously presence of MS omponents, especially obesity. Contemporary hypothesis on the fact that one of the basic factors for prophylaxis of insulin resistance and CD2 is a decrease in the adipose tissue mass was confirmed. Effectively implement: obtained results open the possibility of Vagnosis for developeng new methods of its tieotment.
