Objective of study - to determine the diagnostic and prognostic value of the molecular changes in cancer cells of patients with refractory forms of malignant lymphomas and to develop the methods of treatment depending on levels of these changes. Methods of study - clinical, hematological, morphological, imunohistochemical, radiological, statistical. Basis on the investigation of clinical, hematological, immunophenotype and molecular factors of lymphoma the complex of prognostic criteria (age of patients, stage of disease, ECOG, number of extra nodal sites, bone marrow involvement, expression of РКСв, bcl-2, Ki-67, р-53, GSTP) of the course of disease and programs of differential treatment of non-Hodgkin's lymphoma were invented. According to these data the effectiveness of treatment of non-Hodgkin's lymphoma is 13 % higher when clinical and molecular factors of the disease's course are taking into account. Patients with unfavorable prognosis according to the clinical data and level of expression of РКСв, bcl-2, Ki-67, р53 should be treated with more aggressive therapy (CHOEP) and with favorable prognosis with CHOP.
