The aim of the study was to determine factors causing ineffective hemopoiesis in MDS and contributing to clonal evolution and transformation of the disease into acute leukemia. The obtained results suggest that important role in the course of MDS belongs to the synthesis and interrelation of certain cytokines. Ineffective hemopoiesis is apparently the consequence of the influence of TNF-alfa and probably another cytokine, TGF-beta, which at the late stages of MDS is able to stimulate proliferation of the malignant clones. This process along with angiogenesis activation, occurrence of additional gene mutations during intensified proliferation of genetically modified hemopoietic cells, results in disease progression to acute leukemia. These aspects have to be taken into consideration when managing the patients with MDS.
