In order to fulfill the objectives of the research some parameters of immune status in patients with acute community-acquired pneumonia and COPD were studied. The research was carried out with the help of immunological, serological, clinical and microbiological methods. An inflammatory hypercytokinemia with the concomitant increase in the anti-inflammatory cytokine IL-10 level was observed in all research groups in the first three days of the disease. It was also shown that the local cytokine production data in the course of community-acquired pneumonia of different etiology had more informative value compared to the system serum levels. It was established that the patients with Haemophilus influenzae pneumonia had the highest levels of IL-6 and IL-8. The highest expression level of immunosuppressive cytokine IL-10 was observed on the background of inflammatory process due to the Mycoplasma influenzae infection. For early differentiation between pneumonia of bacterial and viral etiology the determination of bacterial infection markers PCT and sTREM were used. The obtained results were applied for development of the method of efficacy prognosis in antibiotic therapy of patients with infectious diseases of lower respiratory tract.
