Objective: Development and optimization of intensive therapy in various critical conditions, methods of intra- and postoperative analgesia with their cognitivestorage orientation. Methods: general clinic, examinations of various aspects of pain syndrome, tissue metabolism, oxidation-reduction processes, parameters of inflammatory responses, cognitive functions. The results: improved outcomes, reduced mortality by 12.7%, decreased length of stay of patients in the hospital for 3-5 days, the number of postoperative complications in 15.3%, improved quality of life in long term, including by maintaining or recovery of cognitive functions. Novelty: a positive effect of individual drugs on different systems at critical conditions and optimal choices of anesthesia, and their influence on cognitive function. Conclusion: The proposed method of intensive treatment of various critical states reduces the number of complications, mortality, patient day. The suggested methods of anesthesia increases the protection from operative stress and postoperative pain syndrome, and promote preservation of cognitive functions.
