For the first time proved that the clinical course of CDH in infants more difficult in cases of early manifestation from birth. Identified prognostic features that affect the survival of newborns with CDH. It is shown that the main causes of death of infants operated on CDH were strongly pronounced hypoplasia of the lungs with high pulmonary hypertension (50%), generalized infection with multiple organ failure (21.4%), hemorrhagic complications (21.4%) and congestive heart failure (7, 1%). Found a direct correlation between the size of LTR and LHR and the degree of pulmonary hypoplasia and neonatal survival. Elaborated the tactic of pregnancy including indications for its termination. It is proved that the information methods of early diagnosis in postnatal period of CDH are: radiography of the chest cavity, organs of abdominal cavity and stomach with contrast, ultrasound, echocardiography and NSH. Determinate perinatal risk factors in fetuses and newborns with CDH help choose an optimal tactics of pregnancy and therapeutic tactics and strategy in the postnatal period. Were developed algorithm preoperational stabilization. The criteria of readiness newborns with CDH for a radical correction of defects were determined.
