The radiation factor in the development of severe neuropsychiatric disorders. Aim: To determine the effect of whole body external doses on the development of psychoneurological disorders in clean-up workers 1986-1987, using the data of the survivors' registries.Methods: clinical, epidemiological, medical-and-statistical. Results. According to cohort epidemiological study on the data of SRU (68414 men with whole-body external doses) and CER (3548 persons with whole-body external doses) the dynamics of psychoneurological morbidity in the Chernobyl clean-up workers (men) 1986-1987 was identified for the period 1988-2009. Postaccident period was marked by a certain increase in acute and chronic cerebrovascular pathology and mental disorders on the basis of organic brain damage with a maximum incidence observed 12-20 years after the Chernobyl accident. There were determined the significant relative risks for psychotic and nonpsychotic disorders caused by organic brain damage, acute and chronic cerebrovascular pathology mainly at doses over 0.25-0.5 Gy. Younger clean-up workers 1986-1987 were found to be more susceptible to the impact of radiation factor. Using the results of clinical examination the local subregistry (351 people) was developed. Clinical characteristics of the development and progression of demyelinating diseases were provided, main ways of their treatment and rehabilitation being specified. The most unfavourable factors for the development of acute cerebral circulation impairment (ACCI) were: the combination of cerebral arteriosclerosis and hypertension, to a lesser extent - the presence of cerebral atherosclerosis and hypertension separately. Among nonradiation risk factors for the ACCI there were vascular diseases in mother, excess body mass and physical inactivity. In clean-up workers 1986-1987, depressive disorders occur on the background of comorbidity pathology (organic personality disorders, vascular dementia, chronic cerebrovascular and somatic pathologies). The limbic system dysfunction with the possible defeat of the hippocampus is most likely to be pathogenetic mechanism of posttraumatic stress disorder (PTSD) in the Chernobyl clean-up workers. Progression of PTSD among the Chernobyl clean-up workers is complicated by the development of concomitant cerebrovascular disease. Implementation: 11 scientific works were published.
