In this research the clinical monitoring of patients with DM and NAFLD was substantiated, the systemic approach to the improvement of diagnostics and treatment of current category of patients based on determination of integrative synergic changes of morpho-functional state of liver and myocardium implemented. This allowed us to determine the individualization of means of pathogenic correction and prevention of complications. The revealed changes of plasma lipid profile (elevation of cholesterol level, triglycerides and lipoproteins of low density) let us determine the influence of dyslipidemia on clinical course and progression of comorbid pathology of DM type 2 and NAFLD. Derangements of systemic hemodynamics and myocardium contractile function were determined, they were characterized by diastolic dysfunction of left ventricle myocardium (increase of ultimate systolic volume, ultimate diastolic volume and ultimate diastolic size of left ventricle as well). The influence of matrix metalloproteinases as well as their inhibitors on the course and progression of comorbid pathology of DM type 2 and NAFLD was determined. The peculiarities of carbohydrate, protein and pigment metabolism were revealed in patients with combined course of DM type 2 and NAFLD. The peculiarities of morphological changes in liver in patients with DM type 2 associated with NAFLD were studied. On the ground of revealed derangements the schemes of correcting therapy were developed introducing into treatment metabolically active preparations in patients with comorbid pathology of DM type 2 and NAFLD.
